Home Hospitality Application Form
Monday, March 23rd, 2009(Home stay with a Scout Family: 2 Scouts/ Family) Participants can stay at the home of a Scout family for three days before or after the camp but not more than two persons to any family. The program is free of charge and can be arranged prior request at least four (4) months before the jamboree.

Global Development Village Form
Monday, March 23rd, 2009National Scout Organizations are invited to put up an exhibit or activity booth in the Global Development Village (GDV).
In addition to a GDV booth, another booth will be allocated for each NSO in the APR Village devoted for walk-in activities, exhibits to showcase products, crafts and etc. of their own country. Activity booths, to be provided by BSP (2-4mx3m) shall be managed by members of the International Service Team.

REGISTRATION PROCEDURES
For Contingent Support Team
WORKSHEET NO. 1 – CONTINGENT SUPPORT TEAM REGISTRATION
a. Country - Type your country.
b. Country Code - Type your country code
c. National Scout Organization - Type your National Scout Organization
d. National Scout Organization Code - Type your NSO Code
e. Permanent Contact Person - Enter the name of the person that the Jamboree Office could contact should they require any further information.
f. Telephone Number - Enter the contact’s telephone number (including country and area codes)
g. Email Address - Enter the contact’s email address for easy and fast communication.
Registration Data Transfer Log
a. Date - Enter the date of data transfer in the format dd/mm/yyyy.
b. Filename - Enter the filename of the Microsoft Office Excel spreadsheet in the format (Registration Data – Name of Your Country – Date.xls (Date in the format 311209 for 31 December 2009) example “RegistrationData-PHILIPPINES-311209”)
WORKSHEET NO. 2 – CONTINGENT SUPPORT TEAM PERSONAL INFORMATION
a. Individual ID Number - This identification number is allocated automatically to a CST Member when details are entered in the spreadsheet. This cannot be altered by the NSO.
b. Status (Registered, Amended, Deleted) - Select the appropriate status from the list (R = Registered, A = Amended, D = Deleted).
c. First Name - Enter the CST Member’s first name. This should be the name that they are known by.
d. Family Name - Enter the CST member’s Family Name.
e. Date of Birth - Enter the CST member’s date of birth the format dd/mm/yyyy
f. Gender - Select “Male” or “Female” from the drop down list.
g. CST Role - Select the CST member’s job role from the list provided.
h. Jamboree Home Hospitality - Select if the individual will take part in the Jamboree Hospitality (HoHo), and whether this is before or after the Jamboree.
i. Need Visa (Yes or No) - Select “Yes” or “No” from the drop down list depending on whether the CST Member requires a visa to attend the 26th Asia-Pacific Regional Scout Jamboree.
If “Yes” is selected, please complete the data in the next four columns
j. Passport Nationality - Enter the nationality of the CST Member as shown on their passport.
k. Passport Number - Enter the CST Member’s passport number shown on the passport.
l. Date of Issuance - Enter the CST Member’s Date of Issuance of passport.
m. Date of Expiration - Enter the CST Member’s Date of Expiration of passport.
n. Name in Passport (exact name) - Enter the CST Member’s name as it appears in their passport. It is important that this is EXACTLY the same as it appears in the passport.
WORKSHEET NO. 3 – RELIGION AND NEEDS INFORMATION
a. Religion - Select the CST Member’s religion from the drop down list.
b. Religious Diet - Select the appropriate religious diet required for the CST Member from the drop down list.
c. Dietary Need 1 - Enter the dietary need. If the CST Member has more than one, the other should be entered in the next column
d. Dietary Need 2 - Enter the dietary need. If the CST Member has more than one, the other should be entered in the next column
e. Dietary Need 3 - Enter the dietary need.
f. Medical Condition 1 - Please select any medical condition from the drop down list. If the CST Member has more than one, the other should be selected from the drop down list in the next column.
g. Medical Condition 2 - Enter the medical condition.
h. Provide Brief Details - Please provide brief details of the medical condition (if relevant). This field is limited to a maximum of 150 characters.
WORKSHEET NO. 4 – CONTINGENT SUPPORT TRAVEL INFORMATION
a. Arrival Date - Enter the planned arrival date INTO THE PHILIPPINES. This should be entered in the format dd/mm/yyyy.
b. Arrival Time - Enter the planned arrival time INTO THE PHILIPPINES. This should be entered in the format HH:MM (for example 21:30 for 9:30PM), using the 24 hour clock.
c. Arrival Airport/Seaport - Select the planned arrival airport/seaport INTO THE PHILIPPINES from the drop down list. If “other” is selected you will be requested to enter further details in the next column.
d. Arrival Flight/Vessel Number - Enter the planned flight/vessel number (for example PR185)
e. Departure Date - Enter the planned departure date FROM THE PHILIPPINES. This should be entered in the format DD/MM/YYYY.
f. Departure Time - Enter the planned departure time FROM THE PHILIPPINES. This should be entered in the format HH:MM (for example 21:30 for 9:30PM), using the 24 hour clock.
